We are seeking an Registered Nurse (RN) who is passionate about providing high quality, patient-centered primary healthcare and collaborating with a kind team of professionals.

About the Role:

As an RN, you’ll use your critical thinking, good judgement, and interpersonal skills as you identify solutions or approaches to patient problems. You’ll collaborate with clinical staff to design and implement meaningful and individualized patient-centered care. Responsibilities include:

• Performs nursing assessments

• Delivers and coordinates patient care

• Provides patient and family education

• Perform chronic disease management and care coordination

Your primary work location will be our Buffalo locations with travel to all Neighborhood sites in Western New York as needed.

Education and Skills needed:

• Graduated from an accredited two year nursing school

• Current NYS Professional Registered Nurse license

• Current CPR certification

• Highly responsive, timely, accurate and thorough in follow-up and follow-through

• Flexible in managing shifting priorities and timelines

• Excellent oral and written communication skills

What We Offer:

Compensation: $36.50 an hour based on a full-time, 40 hour work week
