Key Responsibilities

• Conduct comprehensive evaluations by reviewing patients' medical histories, performing physical assessments, and determining appropriate treatment plans. Document all findings thoroughly to establish baseline data and goals.

• Communicate evaluation results and treatment plans to patients and their families, ensuring their involvement in goal-setting and treatment processes.

• Implement evidence-based treatment protocols based on assessment findings while monitoring patients' progress and adapting care plans as needed to address any changes in condition.

• Collaborate with other healthcare professionals, recognizing when to engage in interdisciplinary consultations for optimal patient care.

• Manage discharge planning and follow-up care to ensure smooth transitions for patients, documenting all necessary data timely and accurately.

• Maintain efficient time management and productivity, including managing appointments and participating in community-based marketing activities.

• Contribute to departmental goals and initiatives while adhering to all regulations and compliance standards.

• Pursue continuing education and professional growth opportunities, mentoring clinical students as needed.

• Enhance accessibility and customer experience by being adaptable to patient needs and participating in initiatives to remove barriers to care.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
