Description
Qualifications: Graduate of an accredited school of nursing; Licensed to practice as a Registered Nurse in South Carolina; 5 years' experience in an acute care setting; CPR within 1 month of employment. OR Master's degree in social work; 1 year of health care experience with discharge planning or care coordination experience; CPR within 1 month of employment.
Preferred: RN or LMSW with a minimum of 2 years' discharge planning experience in the healthcare setting; ACM or CCM certified.
