Description

American Traveler is seeking a Physical Therapist with home health experience for a 13-week travel contract serving patients across the San Francisco Bay Area.

Details

• Home health setting serving patients across San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo, and neighboring cities
• Day shift, 8:00am–5:30pm, with schedule flexibility based on patient availability
• 13-week contract

Requirements

• Minimum 1 year of home health PT experience required
• Current and active state PT license required (license required if applicable)
• Current BLS/BCLS certification required

Additional Information

• Responsibilities include evaluating and treating patients per medically prescribed physical therapy programs, with a focus on restoring function and maximizing patient performance
• May supervise Physical Therapy Assistants as part of care team responsibilities
• Conducts initial admission and evaluation visits, including OASIS scoring documentation within established timelines
• Documents clinical findings, skilled interventions, goals, and discharge plans for each patient
