Job Purpose / Summary The Hamilton County Supportive Housing Team Leader ensures implementation of daily activities for clients in the supportive housing program. The role encourages client involvement in daily living skills, builds upon clients' strengths with a recovery‑based focus, and ensures activities—both on‑site and off‑site—are community based. Duties Daily Activities Sign‑in to Datis and enter the start time. Check email and review previous day’s progress notes for accuracy and completeness. Complete PM Daily Census Notes. Walk the facility (inside/outside), checking each room for safety and mattress encasements. Check freezers/refrigerators and food storage for organization; assign tasks to staff. Update the menu. Check in with staff on 15‑hour psychosocial education groups and chore activity; ensure clients scheduled for deep clean remain home that day. Facilitate shift‑change report; verify staff are reading pass‑down emails. Discuss morning walk findings with 1st and 2nd shift. Participate with staff in food preparation, cleaning, documentation, and client interaction to ensure quality and consistency. Update client appointment calendar. Screen PRN candidates on the assigned day. Record end‑of‑day time. Weekly Conduct and document weekly supervision with full‑time staff; schedule PRN staff for supervision as needed. Check CSR spreadsheet and update. Create shopping list and pick up orders. Place food bank order and pick up. Report facility issues to Property Manager. Turn in expense requests and update Sun Trust P‑card site. Update treatment plans every 90 days. Ensure staff add documents to the black medical records bag. Ensure 3rd shift staff replace each client’s 15‑hour psychosocial training for the week on Sundays. Monthly Conduct monthly fire drill and vehicle inspection. Update and submit CSRs. Ensure staff schedules are complete for the following month by the 15th. Ensure menu/group activity schedule is due by the 25th. Print and post paperwork by month‑end. Conduct EAP drills every quarter. Additional Job Duties / Expectations Lead hiring and retain staff; maintain appropriate staffing levels. Fill staff vacancies within two weeks of termination. Train staff on program policies and procedures. Submit all concurrent reviews after Services Coordinator’s monthly review. Maintain regular office hours (variances approved by Housing Coordinator). Assist in filling bed vacancies to meet census goals. Complete client treatment and crisis plans; due every 3 months. Ensure supervised housing techs complete shift notes and review accuracy. Complete weekly notes after reviewing all documentation. Ensure supervised housing techs attend quarterly meetings. Ensure Mental Health Techs complete required CARF training modules. Provide direct supervision weekly and address personnel issues. Cover shifts as needed to maintain a 12:1 staff‑to‑client ratio. Act as advocate for supportive housing clients to secure services and reduce barriers. Maintain clinical records per monthly and semi‑annual reporting requirements. Maintain referral records in Athena. Attend weekly team and supervision meetings punctually. Return all texts/calls within 30 minutes of receipt. Respond to all flags, emails, and voicemails within two business days. Ensure daily PM Census (heads on beds) is completed. Submit monthly menu to Housing Services Coordinator by the 25th. Submit supervision logs to HR monthly. Maintain accurate spending records and submit Sun Trust receipts within three days. Assist clients with Social Security office visits within five days of admission. Schedule initial mental health intake within seven business days of admission. Schedule medical/physical within 30 days of admission (or confirm prior 90‑day physical). Apply for food stamps within seven days of admission. Provide an accurate monthly coordinated schedule of activities by the 25th. Provide frontline staff with resources for psycho‑educational groups. Update 15‑hour psychosocial training record every 90 days. Ensure pass‑down log usage and review, confirmed by staff signatures. Send patient health information to medical records daily; request records after each provider visit. Maintain and update Medication Administration Record (MAR). Promote and maintain positive teamwork; organize weekly collaboration meetings. Communicate with PRN staff, providing weekly support. Demonstrate consideration for colleagues and foster harmonious relationships. Maintain positive relationships with internal and external agencies. Communicate directly and approach conflict with problem‑solving methods. Compensation Starting salary: $17.40–$19.85 per hour, based on experience and education. Schedule Flexible hours: minimum 40 hours per week; at least one week per month of 24‑hour on‑call and on‑site holiday coverage, with compensation of nine additional holiday days per year. Travel & Vehicle Maintain a dependable vehicle with valid driver license, F endorsement, insurance, and 3 years of acceptable driving history. Travel clients in personal vehicle or Center van as required. Equipment / Technology Proficiency with computers, Microsoft Office, Excel, virtual meetings, agency portals, cell phone, text messaging, and email. Ability to document in the Electronic Medical Record with correct grammar and punctuation. Experience / Knowledge Minimum two years related work experience. High School Diploma with 10 years of direct care experience, or Bachelor’s degree in health‑related field. Two years of supervisory experience preferred. Skills / Abilities Exposure to biological hazards. Normal hearing; close eye work; lift up to 50 lbs, push/pull up to 150 lbs. Pass drug screen and background check. Certified in verbal and physical de‑escalation techniques, including non‑violent crisis intervention. Manage decision‑making under pressure and demonstrate empathy, flexibility, and effective communication. Location Hamilton County, Tennessee. NHSC Approved Site.
