Work Environment\n

This full-time position allows for flexibility and remote work, providing you with the opportunity to design your work environment to suit your needs while still being an integral part of our team. You'll typically work Monday through Friday, from 9:00 AM to 5:00 PM, which allows for a work-life balance that many professionals in the field strive to achieve. Additionally, you will be supported by advanced telehealth technologies to facilitate efficient patient interactions and documentation.

Key Responsibilities\n

  • Conduct thorough assessments of patients’ mental health status, paying close attention to their emotional, behavioral, and social functioning.

  • Develop, implement, and evaluate individualized treatment plans based on evidence-based practices that align with the cultural background and specific needs of each patient.

  • Provide psychotherapy and mental health services to patients dealing with various conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).

  • Collaborate with an interdisciplinary team, including physicians, nurses, and other healthcare providers, to ensure comprehensive patient care through effective communication and teamwork.

Your primary focus will be on patients receiving hospice care, many of whom are experiencing severe and complex emotional responses to terminal illness. This role requires sensitivity, empathy, and an expertise in addressing issues around grief, anxiety, and the challenges of living with chronic conditions. By providing compassionate counseling and support, you will play a crucial role in enhancing the quality of life for these patients and their families.

  • Master's degree in Social Work from an accredited institution.

  • Current state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
