Benefits & Eligibility Benefits Eligible: Full‑Time Work Shift: Variable – 8 Hour or Less Qualifications License/Certification Required: Current California Registered Pharmacist license Education Required: Pharm.D. degree Inpatient Experience Required: Completed PGY‑1 residency or at least two years clinical hospital pharmacist experience (or current Kaweah Health Pharmacy Resident) Outpatient Experience Required: Completed PGY‑1 residency or at least two years clinical pharmacist experience (or current Kaweah Health Pharmacy Resident) Preferred: Three years of experience overall Specialty Area Requirements (If Assigned) Emergency Medicine / Emergency Medicine Mid‑Shift Pharmacist Specialist: Requires one year of Emergency Department pharmacy experience and/or one year of hospital‑based clinical pharmacist experience in critical care or equivalent setting. Preference for completion of a pharmacy residency program in Emergency Medicine. Infectious Disease Specialist: Requires one year of hospital‑based clinical pharmacist experience. Preference for completion of a pharmacy residency program in Infectious Diseases. Hemostasis Management Specialist: Requires one year of hospital‑based clinical pharmacist experience. Hospital or ambulatory care–based anemia management experience preferred. Home Infusion: Requires Pharm.D. and one year of home infusion pharmacy experience and/or one year of hospital‑based clinical pharmacist experience; or five years or more of Home Infusion Pharmacy experience. Preference for residency completion in home infusion pharmacy. Critical Care Medicine Pharmacist Specialist: Requires one year of hospital‑based clinical pharmacist experience and/or one year in critical care setting. Preference for residency in Critical Care. Ambulatory Care Pharmacist Specialist: Requires one year of Ambulatory Care pharmacy experience and/or completion of residency in Ambulatory Care or General Practice. Strong Ambulatory Care experience and certifications (BCPS, BCPAC, CGP) or eligibility for certification exam preferred. Family Medicine Pharmacist Specialist: Requires residency in General Practice with strong Ambulatory Care experience and certifications (BCPS, BCPAC, CGP). Outpatient Oncology Pharmacist Specialist: Requires completion of PGY‑1 residency or at least two years acute inpatient hospital pharmacist experience. Preference for oncology residency. Job Responsibilities Essential Inpatient Areas: For patients in the acute care setting, the pharmacist reviews orders for appropriateness, profiles/validates and dispenses medications, supplies, or equipment orders accurately and efficiently. The pharmacist follows federal and state law and approved hospital policies regarding medication use, consults with other health care providers to optimize therapy, and manages medication administration safety. They use PYXIS units on nursing units, answer inquiries from physicians and staff, ensure compounded medications meet quality standards, supervise compounding activities, and provide training to pharmacy technicians, interns, and residents. Additionally, they participate in medication reconciliation, medication education, care coordination, and provide interprofessional patient care rounds when applicable. Outpatient Areas: The pharmacist reviews prescriptions for appropriateness, profiles/validates and dispenses medications accurately, follows federal and state law, consults health care providers to optimize therapy, and answers medication inquiries. They train pharmacy technicians and new personnel, monitor patient medication therapies, perform clinical documentation, address drug interactions, give patient dosing information, engage in collaborative patient management, and provide in‑service training when appropriate. Hospital Addendum (If Assigned) When assigned to fill outpatient prescriptions, the pharmacist ensures compliance with federal/state law and hospital policies, counsels patients on use and storage, responds to emergency alerts, prepares IV dilutions, anticipates patient drug needs, fills narcotic requisitions, maintains records, conducts nursing station inspections, oversees drug monitoring programs, consults on therapy, and monitors patients per department procedures. They also price patient charges, provide billing assistance, and manage home infusion duties when applicable. Home Infusion Responsibilities (If Assigned) The pharmacist provides information about products, assists with reimbursement negotiations, maintains inventory and records, purchases on contract, ensures compliance with controlled substance laws, offers after‑hour on‑call coverage, maintains an accurate delivery service, participates in home infusion promotions, and anticipates customers’ needs. Outpatient Oncology Pharmacist Responsibilities (If Assigned to Infusion Center) The pharmacist serves as the clinical pharmacy expert for outpatient oncology services, coordinates medication regimens, reviews and profiles medication orders, collaborates with the Oncology Pharmacist Lead, conducts telephonic and in‑person pharmacotherapy reviews, utilizes the EMR for documentation, identifies cost‑saving opportunities, ensures regulatory compliance, documents interventions, measures clinical/economic effectiveness, participates in formulary processes, and serves as a preceptor for residents and interns. For each specialty, the pharmacist acts as the clinical expert, coordinates medication orders, documents interventions, measures service effectiveness, participates in formulary and policy development, serves as a preceptor, and collaborates with interdisciplinary teams.
