You will document visits using an electronic medical record system, communicate regularly with the broader care team, and participate in care coordination activities to ensure continuity and quality of care. Patient caseloads often focus on adult and geriatric patients with conditions such as post-orthopedic surgery recovery, stroke, cardiac and pulmonary diagnoses, deconditioning, and chronic disease management.
