Home Health Registered Nurse (RN)

Job Description

We are seeking a dedicated and experienced Home Health Registered Nurse (RN) to join our team. The RN will be responsible for providing high-quality care to patients in their homes, ensuring their health and well-being.

Responsibilities

Conduct comprehensive assessments of patients in their home environments.
Develop and implement patient care plans and monitor patient progress.
Perform start of care (SOC), recertification, and daily visits for adults.
Case manage an average of 30 patients, providing necessary support and interventions.
Collaborate with a clinical manager or educator for ongoing support and guidance.

Shift Details

Schedule: Monday to Friday, 8 AM to 5 PM, with weekend rotation.
Shift Length: 8 hours per day.
Holiday rotation may be required.

Qualifications

Must have 2 years of experience as an RN in Home Health or a similar setting.
1 year of experience in a Med/Surg floor within a hospital setting is preferred.
Current RN license in the state of practice is required.
BLS certification (American Heart Association) is required.

Additional Information

Scrubs are the required uniform, with no specific color requirement. Orientation will include two weeks of training with a designated preceptor, which involves several visits to ease the transition into the role. A phone and tablet will be provided for daily use.

Position is open to individuals with a strong background in Home Care and Home Health. Candidates must have a license in hand upon submission. No clinical involvement is required for this role.
