Your mission is to ensure every client receives compassionate, high-quality support and every family feels confident, informed, and cared for. Your responsibilities will include: Developing, maintaining, and overseeing personalized client care plans Building trust and relationships with clients, families, caregivers, and referral sources Mentoring caregivers and supporting meaningful, safe, exceptional care delivery Acting as a calm, positive, solutions-focused leader—no matter how busy the day gets Playing an essential role in helping prospective clients become lifelong Right at Home clients Supervising, on a rotation, our on call care coordinators after hours Supporting community-facing efforts as needed to strengthen our referral partnerships Who Thrives Here?
