Occupational Therapist

Services to be Performed :- Assess and develop interventions for mitigation of fall risks and adaptive equipment including but not limited to the following: • ssess functional mobility to determine needs for mobility equipment • Notify FSH of equipment needed • Provideconsumer/caregiver education on appropriate use and safety with adaptive equipment • Other interventions to mitigate fall risks as recommended Provide additional OT assessments and interventions including but not limited to those related to: Activities of daily living/self-care Fine motor coordination Generalized weakness Muscle re-education Pain management Up to 24 hours per week.
