Family of Caring at Park Ridge is hiring a Director of Social Services (DSS) to lead the social services department at our skilled nursing and rehabilitation center in Park Ridge, New Jersey. This is a full-time Director of Social Services role paying $75,000–$90,000 per year, and we're looking for someone who already knows the job. Prior nursing home experience is required.

Family of Caring is a family-owned New Jersey network of skilled nursing and rehabilitation centers across Bergen County, and Park Ridge is a place where staff, residents, and families know each other by name. Our building offers subacute rehab, long-term care, memory care, respite, and hospice services, plus a dedicated Korean long-term care program. The Director of Social Services is a core member of the interdisciplinary team (IDT) here, and the right Director of Social Services will have real support from an experienced administrator and department heads who've been doing this a long time.

What You'll Do

• Run the social services department for the facility, including admissions psychosocial assessments, care planning, and discharge planning

• Complete the social services sections of the Minimum Data Set (MDS) accurately and on time

• Serve as the main point of contact for residents and families on concerns, grievances, and resident rights

• Participate in daily IDT (interdisciplinary team) meetings, care plan meetings, and Medicare meetings

• Coordinate with hospitals, community agencies, home care, and hospice on transitions in and out of the facility

• Maintain complete, survey-ready documentation and support the facility during New Jersey Department of Health (NJDOH) and Centers for Medicare & Medicaid Services (CMS) surveys

• Supervise social services staff as applicable

Qualifications

• Bachelor's degree in social work or a related field required; Master of Social Work (MSW) preferred

• Current New Jersey social work licensure or certification as required for the role (LSW – Licensed Social Worker, or LCSW – Licensed Clinical Social Worker)

• Must have experience as a Director of Social Services or Social Worker in a skilled nursing facility (SNF) / long-term care (LTC) setting — nursing home experience is a hard requirement

• Working knowledge of the MDS (Minimum Data Set) process, care planning, and federal and state long-term care regulations

• Strong communication skills with residents, families, and clinical staff
