Role – Specialty Physical Therapist – Rehabilitation

Schedule Day Shift

Assignment Details

Contract Length: 13 weeks

Guaranteed Hours: 40

Requirements

• Active license as a Physical Therapist in the applicable state
• Basic Life Support (BLS) certification preferred
• Prior experience in a long-term care or skilled nursing facility preferred

Roles & Responsibilities

• Provide physical therapy services to maximize residents' functional abilities and independence in daily living activities.
• Evaluate residents and establish a therapeutic plan of care with functional goals.
• Consult with referring physicians and healthcare team members on care plans and discharge needs.
• Document all evaluative and therapeutic interactions in the Electronic Medical Record.
• Educate residents and caregivers on therapy goals and procedures.
• Collaborate with other therapy professionals to ensure resident care quality.

Compliance / Notes

Maintenance of required licenses, certifications, and competencies is essential.

Adhere to all infection control procedures.

Participate in department orientation and staff mentorship.
