Description

Resident Care Coordinator

Type: Full-Time Shift/Schedule: Tuesday-Saturday or Sunday-Thursday, Availability for weekend/on-call/call-outs coverage required Location: Avamere at Park Place - 8445 SW Hemlock St., Portland, OR 97223 - Near Tigard, Oregon

Responsibilities:

  • Document information pertaining to changes in residents' status, needs and appointments
  • Ensure proper documentation of all care plans, nurses notes and all other resident correspondence
  • Strong experienced as a Medication Aide
  • Provide orientation and on the job training to Caregivers and Medication Aides
  • Assist with daily living activities (i.e., bathing, dressing, and eating)
  • Measure, record and assist with wellness clinics
  • Conduct weekly, monthly, and quarterly audits
  • Manage scheduling of clinical staff
  • Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy
  • Maintain confidentiality of all resident care in accordance with HIPAA guidelines
  • Complete on-going training as assigned
  • Complete other duties as assigned

Qualifications:

  • Must be at least 18 years of age
  • High-School diploma or equivalent
  • HCA or CNA license preferred
  • Must have at least 1 year of Lead Med Tech/RCC experience in senior living
  • Must have or obtain First Aid & CPR Certification within 30 days of employment
  • Must have or obtain a Food Handlers Card within 30 days of employment
