MDS Coordinator $7500 Sign on Bonus We are seeking a dedicated and experienced MDS Coordinator to join our dynamic healthcare team at Woburn Rehabilitation and Nursing Center. Responsibilities Conduct and coordinate the development and completion of comprehensive resident assessments Conduct resident assessments in accordance with current state and federal rules, regulations, and guidelines Implement resident Care Area Assessments (CAAs) Coordinate and support the development and implementation of individualized resident care plans Collaborate with the interdisciplinary care team to develop comprehensive assessments and care plans Participate in facility survey readiness and throughout the survey process Ensure MDS-related documentation is accurate, complete, and submitted within required timelines Communicate effectively with nursing staff, physicians, therapists, social workers, and other interdisciplinary team members Support compliance with applicable regulatory requirements and facility policies Qualifications Current, unencumbered Massachusetts RN or LPN license Minimum of two years of MDS and PPS experience Previous care planning experience required Strong assessment, documentation, and organizational skills Excellent communication and interpersonal skills Ability to work effectively with an interdisciplinary team Strong knowledge of applicable MDS, PPS, and regulatory requirements Commitment to providing high-quality, resident-centered care What We Offer Competitive MDS nursing wages: $45-$55 per hour Weekly pay! $7,500 Sign-On Bonus 6 paid holidays Medical, dental, and vision insurance Long- and short-term disability insurance Accrued paid time off (PTO) 401(k) On-site employee education and a supportive work environment #J-18808-Ljbffr
