Position Overview and Qualifications

As a Licensed Practical Nurse (LPN) within our Home Health Team, you will be essential in delivering personalized and compassionate care to patients in their own homes. Working alongside Registered Nurses and the care team, you will assist in managing medical needs while fostering patient independence—treating each patient as if they were family during every home visit.

Your key responsibilities will include:

• Administering nursing care at home, which encompasses wound care, medication management, and assistance during medical procedures.
• Accurately documenting assessments, interventions, and outcomes to ensure safe and continuous patient care.
• Monitoring patient conditions and notifying the care team of any changes to facilitate timely and coordinated responses.
• Providing education to patients and their families to enhance medication adherence, improve understanding of health conditions, and encourage greater independence at home.
• Collaborating with RNs, healthcare providers, therapists, and care coordinators to ensure high-quality, individualized care in the home environment.

Qualifications Required:

• Basic Cardiac Life Support certification must be obtained within 15 days of starting employment.
• Valid Driver's License.
• Licensed Practical or Vocational Nurse certification (LPN/LVN).
• Associate Degree or completion of a Vocational School program.

Minimum of 1 year of relevant experience is required.
