Nurse Manager Case Management Overview The Nurse Manager Case Management will oversee and coordinate case management operations within a healthcare organization, ensuring optimal patient care and resource utilization. This role involves collaboration with multidisciplinary teams, including nurses, social workers, and physicians, to facilitate effective care transitions and patient advocacy. The position emphasizes achieving optimal health outcomes, resource utilization, and access to services for patients across various care settings. You will lead and mentor a team of case managers, fostering a culture of excellence and continuous improvement in patient care coordination. The role includes developing and implementing strategies to address social determinants of health and other barriers to care. This is an on-site, full-time position with an opportunity to make a significant impact on community health outcomes. You will work closely with post-acute care providers to ensure seamless transitions and comprehensive care planning for patients. The Nurse Manager Case Management will be instrumental in aligning case management practices with organizational goals and regulatory requirements. Nurse Manager Case Management Key Responsibilities & Duties Lead and manage a team of case managers, ensuring high-quality care coordination and adherence to best practices. Develop and implement care management strategies to address patient needs and improve health outcomes. Collaborate with interdisciplinary teams to facilitate effective communication and care transitions across the continuum of care. Monitor and evaluate care plans to ensure alignment with patient goals and organizational standards. Address social determinants of health and other barriers impacting patient care and recovery. Ensure compliance with regulatory requirements and organizational policies related to case management practices. Provide education and support to staff on case management processes and best practices. Analyze data and metrics to identify trends and opportunities for improvement in care coordination and resource utilization. Nurse Manager Case Management Job Requirements Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Healthcare or related field preferred. Active California Registered Nurse License upon hire is mandatory. Minimum of 5 years of experience in a healthcare role, with 3 years in case management within an acute care setting. At least 1 year of healthcare leadership experience is required. Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred. Strong knowledge of care coordination processes, social determinants of health, and resource management. Excellent communication, leadership, and organizational skills to manage a multidisciplinary team effectively.
