Aspire Physical Recovery Center at Hoover is seeking an experienced RN MDS Coordinator to join our clinical team.

This position requires a nurse who already understands the long-term care environment and the importance of accurate assessment, documentation, care planning, regulatory compliance, and reimbursement.

WHAT YOU'LL DO

• Coordinate and complete MDS assessments accurately and within required timeframes

• Manage assessment schedules and interdisciplinary participation

• Review resident clinical records and supporting documentation

• Ensure diagnoses, treatments, services, functional status, and clinical conditions are accurately reflected

• Monitor changes in resident condition requiring assessment or care-plan review

• Coordinate and participate in interdisciplinary care planning

• Review documentation for accuracy, completeness, and consistency

• Identify documentation gaps and ensure appropriate follow-up

• Support accurate reimbursement through thorough clinical assessment and documentation

• Participate in care conferences, audits, quality initiatives, and survey readiness

• Serve as a resource to nursing staff regarding MDS, care planning, and documentation requirements

• Maintain accurate MDS schedules and required records

WHAT WE'RE LOOKING FOR

• Current Registered Nurse (RN) license in good standing

• Long-term care nursing experience required

• Working knowledge of the LTC resident assessment and care-planning process

• Strong clinical assessment and critical-thinking skills

• Excellent understanding of LTC clinical documentation

• Exceptional attention to detail

• Strong organization and time-management skills

• Ability to manage multiple deadlines and assessment schedules

• Strong electronic medical record and computer skills

• Excellent interdisciplinary communication skills

⭐ MDS EXPERIENCE STRONGLY PREFERRED

Previous experience with MDS, resident assessment, care planning, reimbursement, or clinical documentation in long-term care is strongly preferred.

RAC-CT or other MDS-related certification is a plus.
