RN/ LVN Clinical Coordinator in Boerne, TX

RN/LVN Clinical Coordinator in home health oversees daily clinical operations, 
manages patient schedules, and ensures care aligns with regulatory standards. 
Acting as a liaison between nurses and therapists, they handle physician orders, 
and conduct chart audits. Interaction with administrative and clinical staff 
including front desk, medical records, nursing, and ancillary services (when 
applicable) to ensure maximum utilization of resources and the efficient delivery of 
services.

Coordination & Compliance: Ensuring clinical services are delivered according to 
the patients' care plan and meet state and federal regulations. Reviewing clinical 
notes. 

Communication: Managing referrals, taking verbal or written orders from 
physicians and updating At Risk Registries. 
Software Proficiency: Utilizing industry-standard EMR system like Kinnser/WellSky 
to process.

EDUCATION: RN/LVN Certified

EXPERIENCE: Minimum of 1-2 years of HH clinical experience and Wound Care.

ABILITIES: Must demonstrate strong leadership and effective communications 
skills. Excellent planning, organization and managing skills. Ability to multi-task 
and demonstrates team cooperation. Coordinator will manage Medical supplies 
orders, distributing and inventory. 

Job Type: Full-time Monday through Friday with a rotating on call schedule 
assigned. Traveling may involve from office to supplying storage.
