Responsibilities   As a Physical Therapist with FYZICAL Therapy & Balance Centers, you will provide individualized, evidence-based care while helping patients achieve meaningful improvements in function, mobility, independence, and quality of life.

• Patient Evaluation & Treatment
  • Perform comprehensive evaluations of new patients, including assessment of strength, mobility, balance, motor function, sensory function, functional capacity, and other relevant clinical measures.
  • Develop an appropriate physical therapy diagnosis and identify how the patient's condition impacts mobility, function, participation, and daily activities.
  • Create and implement individualized plans of care based on evaluation findings, patient goals, clinical judgment, and applicable physician referrals or medical information.
  • Provide skilled physical therapy interventions, including therapeutic exercise, functional activities, manual therapy, neuromuscular rehabilitation, patient education, physical agents/modalities, and other appropriate interventions within the PT scope of practice.
  • Continually assess patient response and progress, modifying treatment plans as needed to achieve optimal outcomes.
  • Educate, motivate, and coach patients in their treatment plans, home exercise programs, injury prevention strategies, and techniques for maintaining progress outside of the clinic.
  • Train patients in the safe and appropriate use of assistive or supportive devices when indicated.
• Patient Experience & Plan-of-Care Management
  • Provide comprehensive, compassionate, patient-centered care consistent with FYZICAL's clinical standards and values.
  • Help patients understand their goals, treatment recommendations, expected frequency of care, and the importance of completing their established plan of care.
  • Promote patient engagement and continuity of care by proactively addressing barriers that may lead to cancellations, missed visits, or incomplete plans of care.
  • Maintain an individual appointment-kept rate goal of 90% or greater through patient education, engagement, and effective plan-of-care management.
  • Provide timely care and help maintain an efficient patient schedule while ensuring each patient receives the attention and quality of care they deserve.
• Documentation & Communication
  • Complete accurate, timely, and compliant clinical documentation, including evaluations, daily treatment notes, progress reports, discharge summaries, workers' compensation documentation, incident reports, and other required records.
  • Maintain documentation in accordance with FYZICAL policies and applicable state, federal, payer, and professional requirements.
  • Communicate and collaborate with referring physicians, healthcare providers, case managers, and other members of the patient's care team as appropriate.
  • Support effective communication throughout the patient journey, from evaluation through discharge.
• Teamwork & Clinical Leadership
  • Appropriately delegate patient-care activities to Physical Therapist Assistants in accordance with their competencies, applicable state practice acts, and the established plan of care.
  • Provide appropriate supervision of Physical Therapist Assistants, clinical aides, students, and other support personnel as required.
  • Collaborate with fellow clinicians and team members to provide a coordinated and exceptional patient experience.
  • Assist with scheduling, patient flow, and other clinic functions when needed to support the team and our patients.
  • Contribute ideas and expertise that help strengthen clinical programs, improve patient outcomes, and support the continued growth of our Fishers North and Fort Ben/Lawrence clinics.
• Professional Growth & Standards
  • Maintain the highest standards of professional, ethical, and clinical conduct.
  • Stay current with evidence-based physical therapy practices through continuing education, professional literature, specialty training, and ongoing professional development.
  • Maintain all required licenses, certifications, and professional credentials.
  • Follow FYZICAL policies, procedures, clinical standards, and applicable state and federal regulations.
  • Demonstrate FYZICAL's values through positive communication, teamwork, accountability, professionalism, and a commitment to exceptional patient care.

Required Skills Qualifications & Experience

• Graduate of an accredited Physical Therapy program with a current or eligible Indiana Physical Therapist license .
• Comfortable working independently while also collaborating as part of a supportive, multidisciplinary clinical team.
• Ability to evaluate patients, develop individualized plans of care, and independently progress skilled treatment in an outpatient physical therapy setting.
• Interest in balance and vestibular rehabilitation , with a willingness to develop advanced skills in treating dizziness, vertigo, balance disorders, neurological conditions, and patients at risk for falls.
• Self-motivated, adaptable, and eager to learn new clinical techniques, technology, and specialty treatment approaches.
• Passion for providing individualized, patient-centered care and helping patients improve mobility, confidence, function, and quality of life.
• Strong clinical reasoning, communication, documentation, and patient-education skills.
• Previous outpatient orthopedic, neurological, balance, pelvic, or vestibular rehabilitation experience is preferred but not required .
• Vestibular or balance experience is a plus; training and opportunities for continued specialty development can be provided to the right candidate.
• One or more years of outpatient physical therapy experience is preferred; however, motivated clinicians interested in growing their skills are encouraged to apply.
