Responsibilities include conducting home/community visit observations to assess social workers' unmet training needs, maintaining accurate documentation of QA activities, educating the public and providers about program services, assisting in staffing and program updates, overseeing the QA review process, addressing team education and training needs, participating in special work groups, coordinating program activities for assigned teams, reviewing quality measure standards, identifying deficiencies and providing recommendations, traveling to field offices, documenting workload, and assisting in policy implementation across programs for services to Children with Special Health Care Needs, Newborn Screening, Case Management for Children and Pregnant Women, Personal Care Services, and Community First Choice. Works under limited supervision with latitude for initiative and independent judgment. Performs other duties as assigned. Member of the Incident Command Structure in response to public health emergencies and must comply with applicable policies and procedures. Requires ability to travel throughout Texas and out-of-state. Essential Job Functions Quality Assurance Conducts QA activities to ensure contract compliance and effective resource utilization with contracted entities, provides internal QA reviews, gathers information and prepares corrective action/monitoring plans, reviews quality measures, identifies deficiencies and recommends improvements, conducts QA meetings and provides feedback to supervisors, ensures case managers document activities, and runs ongoing compliance reports. Program & Client Services Serves as Case Management Coordinator for SHSS staff and provides case management support to PCS, CFC, CSHCN-SP, NBS, and CPW programs; assists in training planning and coordination; mentors new case management staff; maintains case files; provides specialized social work consultation and assessment for families of medically fragile children and other identified populations; conducts assessments for PCS/CFC and Birth Defects cases. Community Organization Provides program presentations to community agencies and health entities; participates in public health responses to disaster and emergencies; serves on the Department of Social Work Emergency Response Team. Responds to public health emergencies, performs required training, complies with immunization policy, maintains valid driver license and driving record, and performs other duties as assigned. Knowledge, Skills, And Abilities Knowledge of Case Management principles and practices. Knowledge of children’s programs including Medicaid, CSHCN, CPW, CPS/Foster Care, SSI, NBS, NBHS, Lead, Early Childhood Intervention, CHIP. Knowledge of quality assurance and technical assistance policies and procedures. Knowledge of community resources locally, statewide, and nationally. Knowledge of cultural and socioeconomic issues related to health and disabilities. Proficiency in MS Word, PowerPoint, Excel, Access, Outlook. Ability to compile monthly, travel, and time activity reports. Skill in conducting in-home family needs assessments and developing service plans. Skill in providing presentations and trainings. Skill in conflict resolution, mediation, problem solving. Ability to work effectively as a team member in an interdisciplinary setting. Ability to interpret and apply program policies and procedures. Ability to provide professional leadership. Ability to maintain confidentiality. Registrations, Licensure Requirements Or Certifications Current license to practice Social Work in the State of Texas. Initial Screening Criteria Experience in Case Management principles and practices – 2 years preferred. Experience in developing and conducting training – 2 years preferred. Experience in conducting Quality Assurance activities – 2 years required. Experience in using computers and related equipment including Office Suite, spreadsheets, databases, presentation software – 2 years preferred. Experience in written communications as observed by detail and completeness of information on the state application.
