Licensed Clinical Social Worker

Job Summary The LCSW will provide integrated behavioral health services as a Behavioral Health Consultant (BHC) within a primary care setting, delivering brief evidence‑based assessment and intervention under the Primary Care Behavioral Health model. The role requires successful completion of training phases, collaboration with healthcare teams, and management of a full patient caseload while supporting population health initiatives. Required Qualifications Education: Master of Social Work (MSW) degree from a graduate school of social work fully accredited by the CSWE. License: Have and maintain a current license to practice psychology or license to independently practice clinical social work or licensed professional counselor in any of the 50 states, District of Columbia, Puerto Rico, or U.S. Virgin Islands. Experience: Minimum one year experience as a licensed clinical psychologist, independently licensed clinical social worker, or licensed professional counselor. One year experience within the last three years in the evaluation and treatment of patients with pain, psychological factors affecting their physical condition, or difficulty managing acute or chronic medical problems. Prior training or experience as a behavioral health provider integrated into a PCMH setting is preferred. At least 12 months of hands‑on experience in the last 24 months with computer operations, basic word processing, data entry, and use of an automated medical record system. Proficiency in population health approaches and motivational interviewing is optimal. U.S. Citizenship is required. Key Responsibilities Function as a full‑time provider in a primary care location as a Behavioral Health Consultant, completing Phase I and Phase II BHC training and evaluation per DHA guidelines. Demonstrate core competency skills for BHC independent practice within 90-180 days of starting. Provide focused evidence‑based assessment and intervention (cognitive, behavioral, motivational, acceptance‑based approaches) for presentations such as depression, anxiety, substance misuse, stress, insomnia, pain, obesity, diabetes, tobacco use, and other behavioral health concerns. Deliver care in a population health approach: brief 15‑30 minute appointments, brief episodes of care, same‑day services, educational classes, and team‑based service delivery. Increase referrals from primary care managers, expanding utilization by educating and training PCMH staff and collaborating with the healthcare team. Provide educational or didactic classes (e.g., stress management, sleep enhancement, chronic pain) appropriate for primary care settings. Work independently, communicate effectively with patients and staff, and resolve professional issues. Consult with primary care managers on‑demand, providing same‑day verbal or written feedback. Deliver brief presentations or trainings to primary care team members on behavioral health issues and stepped‑care services. Participate in team efforts to develop, implement, evaluate, and revise clinical pathway programs. Document care and recommendations concisely in the patient’s electronic medical record within the required timeframe. Manage a full daily patient load and maintain scheduled templates per DHA policy. Supervise providers in training or not fully licensed, if needed. Participate in approved research, quality improvement projects, and professional staff meetings as per DHA policies. Attend meetings and trainings related to pain, sleep pathways, BBTI, etc., during normal duty hours. Participate in peer review process. Work Environment & Schedule On site, Monday to Friday 7:30 am to 4:30 pm. Flexible duty hours between 6:00 am to 6:00 pm. Compensation and Benefits $83,000 plus benefits. Physical Requirements Ability to sit or stand for extended periods while delivering brief patient consultations, documenting in electronic medical records, and participating in team meetings; frequent use of standard office equipment. Ability to communicate effectively in high‑stress situations, maintain focus in a fast‑paced clinical environment, and occasionally move throughout the facility and lift up to 10-15 pounds.
