Psych Float Social Worker II/III

At Stony Brook Medicine, Social Workers provide empathic care to patients, families and visitors, applying principles of social casework to deliver counseling, therapy, advocacy, and other services. They plan, coordinate, and document treatment for individuals diagnosed with medical or behavioral health conditions, including substance use disorders, disabilities, violence victims, and other health‑related social issues. Duties Patient and family assessments Person‑centered treatment planning and ongoing counseling with individual and family members to reduce recidivism Documentation and referral processes based on clinical need and regulatory requirements Clinical team case conferencing discussions and collaboration Effectuate discharge planning, including insurance verification, linkage to community providers, authorizations, transportation, and follow‑up calls Work with community agencies for patient post‑discharge needs Supervision of professional social work students Active involvement in performance improvement activities, including data collection and utilization of data for quality improvement Provide coverage in all Behavioral Health and Emergency Department areas with float to mobile outreach team Qualifications Social Worker II – Required MSW from an accredited school NYSLMSW or LCSW At least three years post‑master’s social work experience Minimum 1 year in inpatient acute care psychiatric, mobile team, psychiatric ED or crisis setting Experience with patients experiencing substance use disorder Excellent written, oral, and technical communication skills Ability to work and coordinate with psychiatric/medical treatment teams in an integrated health model Ability to identify and work with high‑risk behavioral health patients Understanding of psychiatric‑specific community resources Strong assessment and psycho‑social skills Knowledge of Long Island and Metropolitan NY community resources Commitment to patient‑first, advocacy, and service excellence Social Worker II – Preferred Experience in substance abuse counseling, psych/medical integration, and behavioral health with adult and pediatric populations Experience in psychiatric emergency room or crisis setting Social Worker III – Required MSW from an accredited school NY SW license within one year of hire Experience with substance use disorder patients Excellent written, oral, and technical communication skills Coordination with psychiatric/medical treatment teams in integrated model Identify and work with high‑risk behavioral health patients Understanding of psychiatric‑specific community resources Strong assessment and psycho‑social skills Knowledge of Long Island and Metropolitan NY community resources Commitment to patient‑first, advocacy, and service excellence Social Worker III – Preferred Experience in psychiatric emergency room or inpatient setting Experience in substance abuse counseling, psych/medical integration, and behavioral health with adult and pediatric populations Shift hours: 8 am‑6 pm, 10 am‑8 pm, 12 pm‑10 pm, 2 pm‑12 pm Benefits We offer a competitive salary range of $86,162–$109,275 base, with location pay of $4,000 for certain positions, and additional benefits including generous leave, health plans, and state pension.
