Work Environment

You will work remotely from Albany, New York, collaborating with a network of healthcare professionals dedicated to providing comprehensive support to patients in hospice care. Key Responsibilities

• Conduct comprehensive biopsychosocial assessments that are age and culturally responsive to identify the mental health, substance abuse, and medical needs of patients within the hospice setting.
• Deliver a range of behavioral health services, including intake evaluations, individual and family counseling, crisis intervention, and treatment planning, ensuring that care is tailored to each patient’s circumstances.
• Document progress notes and assessment findings electronically, adhering to organizational policies and procedures for confidentiality and data management.
• Facilitate effective communication between patients and their healthcare providers, including timely referrals for additional treatment and support services as needed.
• Develop and maintain relationships with local resources and community agencies, providing patients and their families with access to medication assistance programs and other relevant support services.
• Engage in collaborative efforts with Quality Improvement teams to enhance service delivery and implement processes for data collection and analysis.

Patient Population / Client Focus

This position focuses on serving patients in hospice care, including individuals and families grappling with the emotional complexities of terminal illness. Your expertise will be crucial in addressing psychological distress, facilitating family dynamics, and identifying potential crises that require immediate intervention. Qualifications

• Active and valid license as a Licensed Clinical Social Worker (LCSW) in the state of New York.
• Master’s degree in Social Work from an accredited institution, coupled with a minimum of three years of post-licensure clinical experience.
• Strong knowledge of therapeutic techniques, crisis intervention strategies, and the ability to make independent clinical judgments related to counseling and support services.

Preferred Experience

• Proven experience in group facilitation and planning processes, with a track record of successful intervention and support in diverse team settings.
• Familiarity with electronic health records (EHR) systems for effective documentation and patient management.
• Experience working within hospice or palliative care environments is highly desirable.

Compensation and Benefits

The position offers a competitive annual salary in the range of $80,000 to $100,000, commensurate with experience and qualifications. Additional benefits include:
