Salary Range: $86000 $105000 annually (commensurate with experience)

Sign-On Bonus: $12000

The Opportunity:

Your Role and Impact:

• Serve as a primary coordinator of all care for hospice patients ensuring seamless and compassionate end-of-life care.
• Conduct comprehensive assessments and create personalized care plans in collaboration with patients families and the interdisciplinary team.
• Provide expert nursing care symptom management and emotional support tailored to each patients needs.
• Act as a liaison advocating for patients and families needs and coordinating with healthcare providers and community resources.
• Educate families and caregivers on end-of-life care ensuring they feel supported and informed.

Who You Are:

• A licensed Registered Nurse (RN) in Massachusetts with experience in hospice or palliative care. preferred but not required.
• Compassionate with a deep understanding of the emotional complexities involved in end-of-life care.
• An exceptional communicator and a strong advocate for patient and family needs.
• A team player who collaborates effectively with interdisciplinary teams.
