Remote RN Care Manager - Member Care & Travel

Humana

Humana, in collaboration with Inclusa, is seeking a Registered Nurse in Wisconsin to join our member care team. This remote role requires travel up to 40% to visit members, assess health and safety needs, and develop comprehensive MCPs while coordinating with acute and primary care providers. You will document thoroughly within DHS timeframes, conduct monthly member contacts, and support new assignments. The position follows a Monday–Friday schedule with standard CST hours.
