Physician Assistant or Nurse Practitioner - Family Practice

Job Description REPORTS TO : Medical Director / Practice Manager PAY RANGE : $99,200 - $123,000 per year WORK SCHEDULE : Full-time (40 hours). Weekdays only. No evenings, weekends, or on-call. POSITION SUMMARY : Provides Family Practice services to individuals of all ages. Primary Responsibilities Conducts initial examinations with recommendations for treatment and follow-up care to individuals. Conducts follow-up examinations with recommendations for any changes in the care plan. Prescribes medications as condition indicates within the appropriate scope of licensure. Make appropriate referrals when necessary. Discusses the medical area of the treatment plan with the individual, parents, relatives/advocates, or guardians. Serves as a member of the interdisciplinary clinic team to ensure that the medical needs of patients are met. Maintains confidential medical records. Ensures referral and/or consultation with a collaborating physician, as appropriate. Assures compliance with all regulations of health and clinical services. Ensures standards of medical care. Assists nursing staff in providing emergency first aid, crisis intervention, and infection control procedures. Ensures review of medical care provided and recommended by collaboration physician. Performs any other duties agreed upon with the Medical Director. Requirements Current, unrestricted New York State licensure as Physician Assistant or Nurse Practitioner Current Board certification At least one year of experience as PA or NP preferred Benefits Community Health Center of the North Country offers eligible employees excellent, affordable benefits options.
