Virtual Family NP - Wellness & Care Coordination

Jobtailor

Jobtailor seeks a Board Certified Family Nurse Practitioner to conduct virtual wellness visits for adults and children. The role requires active licensure in TX/LA/OK and experience with wellness assessments and care coordination.

You will work remotely with a clinical in-home team and must communicate effectively while managing documentation in HIPAA-compliant platforms. Strong problem-solving and independent/team collaboration will be essential as you provide preventive care interventions,
