Position Overview The RN Case Manager assesses plans, coordinates care, evaluates and advocates for services to meet patients health needs as they move through the continuum of care to promote quality and cost‑effective outcomes. Benefits Eligibility Full‑time benefit eligible Work Shift: Day–8Hour or less Shift (United States of America) Qualifications Must hold an active California RN license Must have Basic Life Support (BLS) certification Preferred education: Bachelor of Science in Nursing (BSN), Master of Science in Nursing (MSN), or currently enrolled in an RN‑BSN program Required experience: Two years of acute care clinical nursing experience (LVN/RN) Preferred experience: RN experience in general acute care Mandatory requirement for Emergency Department positions: Must have at least three years of RN experience in an Emergency Department or Critical Care setting. Job Responsibilities Assessment: Acts as a resource person for clinical care issues, identifies action plans, and facilitates communication with appropriate physicians. Assists and communicates with physician offices and all appropriate departments regarding new admissions, demographic information, and other data pertinent to the patient/family that may affect care. Evaluates the assessment process of new patients within 30days of admission to determine needs and develop a Care Management Plan addressing Social Determinants of Health (SDOH) barriers. Planning: Establishes individualized care plans with specific action steps for each patient. Collaborates with patients/families, care teams, and physicians to determine goals and objectives for improved patient outcomes. Takes the lead in assessing care‑plan progress and revises it as necessary. Rounds with physicians and multidisciplinary teams and coordinates with the team to ensure graduation‑planning goals are developed and modified as needed. Implementation: Takes the lead in moving patients through the continuum of care in a timely, cost‑effective, and safe manner. Assists in organizing and integrating resources needed to meet stated goals and plans. Works with patients, families, multidisciplinary teams, and external services to accomplish outcomes. Supervises implementation of the treatment plan, including appropriate use of pre‑printed orders, documents progress notes in the electronic health record, and makes timely referrals for services. Evaluation: Evaluates the care plan for appropriateness, monitors progress toward outcomes, and suggests appropriate levels of care when changes in function, medical, or psychological status occur. Reviews medical records for proper documentation of services and evidence of progress. Collaboratively recommends care‑plan changes as needed. Professional Mandates: Practice is guided by the Code for Nurses, ensures decisions and actions are ethical, maintains patient confidentiality within legal mandatory mandators, acts as an advocate for self‑advocacy, and delivers non‑judgmental and nondiscriminatory care that respects patient autonomy, dignity, and rights. #J-18808-Ljbffr
