Field-Based Case Manager: Community Care & Support

Humana

Humana, in partnership with Inclusa, seeks a Case Manager to serve Wisconsin Family Care members in Richland and Iowa Counties. Reporting to the Manager of Care Coaching, you will conduct comprehensive assessments, develop a Member Care Plan, coordinate with a Field Care Nurse, and support non-medical needs. This full-time role requires a Bachelor's degree in human services or related field (or 3 years with other degree), a valid driver's license, and willingness to travel up to 40%. #J-18808-Ljbffr Humana
