As Clinical Manager, you will provide leadership and support to our Clinical Supervisors and clinical staff while helping ensure safe, high-quality, individualized care for patients in their homes.

What You’ll Do

• Provide leadership, guidance, and support to Clinical Supervisors and clinical staff.

• Coordinate patient care, staffing, and clinical services from admission through discharge.

• Evaluate referrals and provide clinical recommendations as directed.

• Conduct supervisory visits and assess patients, families, home environments, and clinical staff.

• Provide training, education, mentorship, and performance support to clinical team members.

• Promote compliance with CHAP standards, state and federal regulations, and Aveanna policies .

• Partner with case managers, discharge planners, and other healthcare professionals to support continuity of care.

• Identify opportunities to improve patient outcomes and reduce clinical risk.

• Support hiring, retention, and professional development of clinical staff.

• Participate in on-call responsibilities as assigned.

• Serve as the clinical lead in the absence of the Clinical Director.

• Provide direct patient care when clinically necessary.

What We’re Looking For

Required:

• Diploma, Associate’s, or Bachelor’s degree in Nursing from an accredited program.

• Current RN license applicable to the state(s) of practice.

• Home care experience and OASIS experience.

• Clinical experience as required by applicable state and federal regulations.

• Valid driver’s license and acceptable MVR.

• Ability to travel throughout the assigned service area.

Preferred:

• 2+ years of Clinical Supervisor or clinical leadership experience.

• Experience leading, mentoring, or developing clinical professionals.

• Experience caring for medically complex patients and their families.

• Strong understanding of clinical compliance and regulatory requirements.

Physical & Environmental Requirements

This position requires travel throughout the assigned service area and the ability to perform clinical and supervisory responsibilities in patient homes. Duties may include prolonged sitting, standing, walking, bending, reaching, kneeling, climbing stairs, and occasional lifting, carrying, pushing, or pulling of up to 40 pounds.

The role may involve exposure to allergens, blood, bodily fluids, infectious diseases, and other conditions associated with providing care in the home.
