Rehabilitation Specialist – Full Time, Springfield, MA Requisition ID: 2978 Salary Range: $26.00 – $26.00 per hour Hours: 40 hours per week (Monday-Friday 8:00 am – 4:00 pm). Rate of Pay: $26.00 per hour. Sign‑on bonus: $500 (split as $250 at the completion of first week of training and $250 after successfully completing first 90 days). Location: Springfield, MA Responsibilities Gather data, document, and present each member’s goals and objectives in accordance with ISP regulations; coordinate services with case managers and parent/guardian of program members including goals, issues, concerns, and abilities; ensure the provision of high quality services. Oversee screening and prioritizing of referrals; document member progress; implement behavior management strategies; investigate incidents and perform all other assigned duties. Qualifications High school diploma with 5 years related experience OR associate’s degree with three years related experience OR bachelor’s degree with one year related experience. Knowledge of and respect for diverse populations. Previous experience working in human services preferred. Requirements 2 years of prior case management or care management experience. Management experience required. Proficient computer skills, including Microsoft Word, data entry, e‑mail, and internet. Organized, detail‑oriented, strong time‑management skills, able to work in a fast‑paced multi‑tasking environment. Demonstrated ability to deliver high level of customer service/satisfaction for internal and external customers. Excellent communication, interpersonal, and listening skills. Valid driver’s license, good driving record, ability to use vehicle and willingness to transport participants in personal and/or agency vehicle. Benefits Health, dental, and vision insurance plans.
