Transitions At Home is currently seeking a Full-Time, compassionate and experienced Physical Therapist to join our home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring patients receive the highest quality of service and support throughout their recovery journey. What does a Transitions at Home Physical Therapist do? As a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.
