• Coordinate and manage the care of hospice patients, ensuring personalized, patient-centered, and compassionate care is delivered.
• Provide direct nursing care, including pain management, symptom control, and emotional support to patients and families.
• Serve as the primary point of contact for patients, families, and the healthcare team, ensuring clear and consistent communication.
• Conduct regular assessments of patients' conditions, adjusting care plans as needed to meet changing needs.
• Ensure compliance with hospice regulations and standards, maintaining accurate and timely documentation.

Candidate requirements

• A bachelor of Science in Nursing (BSN) is required; an Associate Degree in Nursing (ADN) with relevant experience will be considered.
• Minimum of 3 years of nursing experience, preferably in hospice, palliative care, or home health.
• Basic Life Support (BLS) certification is required.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.
