Overview

Position Title: Social Worker - MSW / LSW

Location: Riley Hospital

Shift: Full-Time Days

Position Overview: The social worker provides face-to-face assessments, discharge planning, resource coordination, crisis intervention, and clinical support for diverse patient populations. This role emphasizes collaboration with the healthcare team, supporting patients and families through complex psychosocial needs, safeguarding, and care transitions. Roles and Responsibilities:

Meet face-to-face with patients and families to assess psychosocial needs.

Collaborate with healthcare team members to ensure safe, timely, and effective care and discharge planning.

Interpret and communicate complex patient and family needs, role modeling caring practices.

Assess psychosocial risk factors based on prior functioning, support systems, and reactions to illness.

Counsel patients and families on emotional, social, and financial impacts of illness or disability.

Access and mobilize family and community resources to meet patient needs.

Conduct mental health assessments upon physician referral and provide therapeutic interventions.

Serve as a primary liaison for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence, coordinating with protective agencies.

Maintain principles and practices of crisis intervention and de-escalation.

Provide intervention in guardianship, foster care, adoption, and mental health Level II placements.

Deliver specialized mental health services, including crisis intervention, addiction assessments, and psychiatric recommendations.

Support safety in dangerous or potentially violent situations using de-escalation techniques.

Support patients during end-of-life, guardianship, foster care, and adoption processes.

Document all clinical activities accurately in electronic medical records.

Develop and maintain collaborative relationships with hospital staff, law enforcement, community agencies, and protective services.

Qualifications & Requirements:

Master's Degree in Social Work (MSW) required.

Permanent licensure in Indiana as an LSW.

Certification in Crisis Prevention Intervention or an equivalent de-escalation training within 6 months of hire.

Knowledge of evidence-based psychosocial assessment, short-term counseling, and crisis intervention for mental health and substance use disorders.

Understanding of disease processes and human behavior related to social and health needs.

Complex discharge planning skills for safe patient transitions.

Knowledge of state and county legal mandates for protective services.

Understanding of growth and development principles across the lifespan, including end-of-life care.

Ability to identify decision-makers and coordinate goals of care conversations.

Experience adapting services for geographically, culturally, and financially diverse populations.

Ability to develop strong working relationships with hospital administrators, medical teams, law enforcement, community agencies, and protective services.

Computer literacy and proficiency in electronic medical record documentation.

Ability to apply crisis intervention and de-escalation techniques effectively.

May be scheduled for on-call and holiday rotations.
