• Provide direct patient care to residents in our skilled nursing facility, including medication administration, wound care, and vital sign monitoring

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Conduct vital sign checks and document resident data accurately

• Report changes in resident condition to the registered nurse or physician

• Participate in quality improvement initiatives to enhance the overall resident experience

Requirements

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Oklahoma

• High school diploma or equivalent required; associate's degree or higher in nursing or a related field preferred
