RN Care Coordinator

Job Summary and Responsibilities Qualified external applicants with at least six months of direct RN experience are eligible for a $20,000 sign‑on bonus! RN Case Managers may be eligible for a relocation bonus up to $8,000. The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. This role supports patients’ health needs while promoting quality of care, cost‑effective outcomes, and compliance with hospital policies, standards of practice, and Federal and State regulations. Emphasis is on care coordination, communication and collaboration with utilization management, nursing, physicians, ancillary departments, insurers and post‑acute service providers to progress care toward optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for the patient and family by addressing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions. The position strives to enhance the patient experience. Work Schedule: Monday – Friday 11:00 am – 7:00 pm. Job Requirements In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: Graduate of an accredited school of nursing BSN preferred; Nursing Diploma or Associate’s Degree in Nursing required RN licence in the applicable state of employment required Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or nursing field in lieu of acute experience At least five years of total nursing experience required Certified Case Manager (CCM), Accredited Case Manager (ACM‑RN), or UM certification preferred Experience working with EMR preferred Working knowledge of regulatory requirements and accreditation standards strongly preferred BLS from the American Heart Association required within three months of hire if located within hospital Physical Requirements – Medium work: exert up to 50 lbs force occasionally, up to 20 lbs frequently, and/or up to 10 lbs constantly. Where You'll Work Located in Lakewood, Colorado, St. Anthony Hospital is a Level I Trauma Center providing a full range of medical specialties and health‑care services to Denver and the surrounding region.
