- Develop individual rehabilitation plans alongside Rehab Team
- Review and approve monthly progress reports for Psychiatric Rehabilitation Program (PRP) clients.
- Conduct an initial and periodic review of clients existing Individual Rehabilitation Plan (IRP) with Program Director and assigned Direct Care Staff member. Develop new IRPs if needed
- Review and approve contact notes from Direct Care Staff members to monitor client progress, and identify clients that require referrals for additional treatment
- Assist Program Director in identifying staff training needs
- Regularly review and audit documentation for quality assurance
- Provide supervision and direction over the PRP program
