Occupational Therapist

Occupational Therapist Profession: Occupational Therapy Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 1+ years of experience in occupational therapy License: Active Occupational Therapy license required Must-Have: - Long Term Care or Rehabilitation experience - Level II fieldwork experience for new graduates Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders. Administers various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, and modalities. Utilizes supportive and assistive devices as necessary. Assesses patient's progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Communicates effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Prepares comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all required information in the electronic documentation system. Maintains patient records according to company policy. Records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Engages in facility and company required training, in-services, and conferences. Obtains necessary DME or supplies as per regulatory and clinical practice requirements. Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students. Adheres to practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
