Part-Time | Outpatient (OP) & PRN Skilled Nursing Facility (SNF/OP)

Position Summary

Trinity Rehab is seeking a licensed Physical Therapist (PT) to join our team in Asheville, NC. This part-time position offers a combination of outpatient services and PRN coverage in a skilled nursing facility (SNF) setting. The ideal candidate is compassionate, motivated, and committed to delivering high-quality patient care.

Key Responsibilities

• Evaluate, develop, and implement individualized treatment plans based on physician referrals
  
• Provide skilled physical therapy services to patients in both outpatient and SNF settings
  
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
  
• Maintain accurate and timely documentation in compliance with regulatory and company standards
  
• Collaborate with interdisciplinary teams to ensure coordinated patient care
  
• Supervise and provide direction to Physical Therapist Assistants (PTAs) and therapy aides, as applicable
  
• Educate patients and families on treatment plans, goals, and home exercise programs

Requirements

• Current North Carolina Physical Therapist (PT) license in good standing
  
• Graduate of an accredited Physical Therapy program
  
• Strong clinical skills and ability to work independently across multiple settings
  
• Excellent communication and organizational skills
  
• SNF experience preferred, but not required
