Hospice RN Case Manager

$40 - $55 per hour

Overview Croí Healthis is a mission‑driven, non‑profit organization providing home health, palliative, hospice, and wellness services across 27 communities in Massachusetts. We are looking for an experienced, talented, and compassionate Hospice RN Case Manager for a full‑time position (Monday‑Friday, 8:00 a.m. – 5:00 p.m.). What you will be doing The hospice RN case manager will coordinate and provide the delivery of hospice services to patients and clients of the agency through the application of the nursing process and by following the agency’s philosophy, mission, and standards. The role requires strong clinical assessment and prioritization while working as a team player with patients, colleagues, and families. In addition, the case manager will communicate effectively and provide medication and symptom management. Compensation and Benefits Pay rate: $40/hr – $55/hr, based on experience.
