Physical Therapist – Home Health | Tru Healthcare

Up to $115,000 Base Salary + Bonus | New Grads Welcome | No Weekends or On-Call

What We Offer

• Competitive salary: Up to $115,000, depending on experience
• Annual productivity bonus: Potential to earn up to $16,000
• Sign-on bonus: $5,000 for experienced PTs or $1,000 for new graduates
• Mileage reimbursement: $0.56 per mile

What You'll Do

• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Provide physical therapy services primarily to geriatric patients in their homes.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and families on treatment plans and strategies to support recovery and independence.
• Collaborate with nurses, occupational therapists, physicians, and other care team members.
• Complete accurate and timely clinical documentation.
• Support start-of-care assessments and communicate changes in patient needs to the care team.

What We're Looking For

• An active Physical Therapist (PT) license.
• Ability to manage a patient caseload and work independently in a home health setting.
• Strong communication, organization, and time-management skills.
• Home health, OASIS, or geriatric experience is a plus, but not required.
• New graduates are welcome! Training and support are provided.

Service Area

You'll primarily serve the Southern and Western Twin Cities Metro, extending approximately from St. Paul to Minnetonka and south to Apple Valley. We aim to keep clinicians within a primary territory and minimize driving through thoughtful scheduling.
