Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona state PT license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in outpatient physical therapy settings preferred.
• Demonstrated ability to complete comprehensive evaluations, develop and implement individualized treatment plans, and progress patients toward discharge within established lengths of stay.
• Proficiency with documentation including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in compliance with state and federal guidelines; experience with electronic medical records preferred.
• Ability to supervise and delegate to discipline-specific therapy staff and to participate in student clinical education when assigned.
• Maintain professional licensure and meet state-mandated continuing education requirements.
• Adherence to professional codes of conduct, patient confidentiality, and safety policies.
• Comfort working in a rural clinic environment.

• Shift pattern: 5x8 schedule.
• Assignment duration: 13 week assignment; client indicates a 6 month duration is desired.

• Candidate may be expected to support clinic safety and performance improvement initiatives and participate in student supervision and education.
