PRN Occupational Therapist - Home Health

Location: Based in Youngstown, OH (Travel required) Employment Type: PRN (As Needed) Pay Rate: $50–$60/hour (Based on experience, visit type, and travel requirements) Position Summary We are seeking a compassionate, self-motivated PRN Occupational Therapist to join our home health team. This role is ideal for a therapist who enjoys personalized, one-on-one patient care and is willing to travel beyond the Youngstown area. As a PRN OT, you'll be responsible for assessing patient needs, developing treatment plans, and delivering occupational therapy services to help patients maintain or improve their ability to perform daily living and functional tasks in their home environment. Responsibilities Perform in-home OT evaluations and reassessments Develop and implement individualized treatment plans to promote independence and function Instruct patients and caregivers on home-based exercises, safety, and adaptive strategies Document services accurately and promptly in EMR systems Communicate and coordinate with physicians, PTs, nurses, and other care team members Travel to homes throughout Central and Northeast Ohio, including Mahoning County Requirements Current Ohio Occupational Therapist (OTR) license in good standing At least one year of experience in home health, outpatient, or inpatient settings preferred Valid driver’s license, reliable vehicle, and auto insurance Ability to work independently and manage a flexible PRN schedule Willingness to travel longer distances as needed Preferred Qualifications Experience working with adult and geriatric patients Familiarity with Medicare documentation Proficiency with EHR systems What We Offer Hourly rate: $50–$60/hour (depending on experience) Flexible, self-determined scheduling Mileage reimbursement beyond local territory Responsive clinical support team Opportunity to make a meaningful impact through one-on-one patient care #J-18808-Ljbffr
