Impactful Crisis Services Case Manager

Community Bridges, Inc. (CBI) is seeking a Familiar Face Case Manager to coordinate care and discharge planning for individuals utilizing crisis services. The role focuses on preparing patients for clinically appropriate services and coordinating with all involved parties to reduce barriers to treatment. The position reports to the Clinical Director.
