Benefits Actual Work/Life Balance Competitive Pay $75,000 to 88,000 annual salary (commensurate with experience) + bonus opportunities Benefits Package including Medical, Dental, and Vision insurance Paid Time Off with accrual starting on day 1 401 (k) plan with employer match and 100% vesting after 90 days of employment A culture with an emphasis on appreciating and valuing the team member The opportunity to be part of a rapidly growing national company, with possible position upgrades Responsibilities The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psychosocial assessments, interventions, and knowledge of community resources. Work closely with patients on a 1-1 level but receive strong support from a patient care team. Manage day-to-day activities including phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Demonstrate time management skills to manage visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns. Demonstrate ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, have a valid driver’s license, and auto insurance to travel across the service area. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership Ability to work in a 1-1 setting with patients while receiving strong support from a patient care team Ability to work independently with day-to-day activities including phone, email, and written communication; participate in meetings with healthcare professionals; and understand hospice rules and regulations Time management skills to manage visits, documentation, and meetings; prioritize and triage multiple concerns Ability to learn and use an electronic medical records system Reliable transportation, valid driver’s license, and auto insurance required to travel across the service area #J-18808-Ljbffr Suncrest Hospice
