This role will focus on Pulmonary Hypertension/Advanced Heart Failure. Previous experience is required***JOB SUMMARYOhioHealth is seeking a pharmacist to join the Heart Disease Management team. This pharmacist will provide evidence-based therapy management and care for patients with pulmonary hypertension as well as other heart conditions (heart failure, LVADs, etc). They will work alongside an interdisciplinary care team in to provide safe, high quality, and cost-effective drug therapy. The pharmacist is responsible for patient-centered education throughout the course of their therapy, providing appropriate patient instruction and dosing guidance based upon tests and laboratory results, reviewing individualized patient treatment plans, and ensuring patient adherence to medication therapy. This position will support primarily ambulatory needs but have limited inpatient responsibilities related to transitions of care and/or medication use work. Pharmacists will be expected to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.SPECIALIZED KNOWLEDGEOptimal candidates will have a PGY2 in ambulatory care and BPS certification in ambulatory care, cardiology, or pharmacotherapy or equivalent clinical experience.MINIMUM QUALIFICATIONS - Bachelor of Science or Doctor of Pharmacy degree from an accredited institution- Current Ohio pharmacist license- Basic Life Support Certification—current or obtained during the orientation period (unless greater than 50% of scheduled work hours, as determined by the manager, are completed off-site or not in a patient care setting) SPECIALIZED KNOWLEDGE- Pharmacokinetic drug dosing experience- Drug therapy managementDESIRED ATTRIBUTES- ASHP accredited residency or equivalent pharmacy practice experience- Pharmacy Board Certifications or other certifications- Advanced-level degree in healthcare field- Collaborative practice experience- Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)Responsibilities And Duties:Conducts essential responsibilities in a manner consistent with OhioHealth’s Service Excellence Standards of Behavior and Core Values: Compassion, Excellence, Inclusion, Integrity, and Stewardship. Utilizing data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient outcomes. Integrates medication improvement initiatives, activities, and changes with clinical and operational department programs and committees.
