The Center for Key Populations (CKP) at Community Health Center, Inc. seeks a Prevention Health Navigator in Middletown, CT to advance PrEP outreach, facilitate access, and provide direct linkage to care for individuals seeking PrEP services. You will educate communities about PrEP, offer navigation for insurance enrollment, provider identification, scheduling, and cost management, and maintain confidential patient records in an EHR system, supporting CKP staff in HIV prevention. #J-18808-Ljbffr Community Health Center, Inc
