Job Summary and Qualifications

We are currently looking for an ambitious Transplant Pharmacist to join our team. As the Transplant Pharmacist you will be responsible for participating and leading pharmacy clinical programs in the area of Transplant services. This includes monitoring patient medication regimens and providing appropriate interventions, interacting with all medical and surgical specialties responsible for the care of transplant patients and collaborating with nursing staff to ensure optimal care of the transplant patient. What you will do in the role:

• You will coordinate and assist with the development, implementation and maintenance of clinical pharmacy programs to ensure clinical services meet the requirements and needs of the patient population and community.
• You will provide comprehensive pharmaceutical care to transplant recipients in a culturally competent manner to include evaluation, identification and solution of medication related problems for transplant recipients.
• You will prepare and assist with discharge planning for all transplant recipients.
• You will evaluate transplant recipient medication regimen on a regular basis.
• You will provide clinical pharmacy support and execute clinical pharmacy programs for all patient areas.

What qualifications you will need:

• Bachelors of Science or Pharm D degree in Pharmacy from College of Pharmacy
• Completion of PGY-2 pharmacy residency in transplant pharmacy from an ASHP accredited resident program preferred
• Minimum of 2 years work experience in an acute care hospital setting with significant exposure to the transplant or critical care environment
• Current FL Pharmacist license
