You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapy to improve strength, balance, mobility, and functionMonitor patient progress and adjust treatment plans as neededMaintain accurate and compliant clinical documentationCommunicate effectively with physicians, residents, families, and care team membersParticipate in care planning and discharge planningProvide education and guidance to residents and staffQualificationsDegree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)Active, unencumbered Physical Therapist license in South Carolina (required)Must maintain all continuing education and remain in good standing with the State BoardExperience in a skilled nursing or rehab setting preferredFamiliarity with Net Health and PointClickCare (PCC) preferredStrong clinical, communication, and organizational skillsCPR certification (if required by licensure)Why Join Greenville Post Acute?At GPA, you’ll enjoy the flexibility of PRN work while being part of a team that truly cares about residents and each other.
